Services for Bellevue City Residents

IMMUNIZATIONS

Please call 1-888-694-2443 (Ext. 241) or 419-668-1652 (Ext. 241) to make appointments for immunizations. Immunizations will be given at the Bellevue Health Department, 3000 Seneca Industrial Parkway, Bellevue, Ohio. Immunizations are $10 per shot and Medicaid is accepted. No child is turned away for inability to pay.

Records for clients who received services at the Bellevue City Health Department will be available at the Huron County General Health District located at 180 Milan Avenue in Norwalk.

BIRTH AND DEATH CERTIFICATES

Birth and Death Certificates are available at the Huron County General Health District, 180 Milan Avenue, Norwalk, Ohio 44857. For assistance, please call the Vital Statistics Office at 1-888-694-2443 (Option 5) or 419-668-1652 (Option 5).

Certificates may be requested by phone, by mail, or in person at the Norwalk office. Certified copies are $20 per copy. Office hours are Monday-Friday, 8:00 a.m. – 4:30 p.m.
