Please call 419-483-3404 or 419-668-1652 (Ext. 241) to make appointments for immunizations. Immunizations are given at the Health Department, 3000 Seneca Industrial Parkway, Bellevue, Ohio by appointment. Immunizations are available for children and adults.  Medicaid and private insurance are accepted. No child is turned away for inability to pay.

Records for clients who received services at the Bellevue City Health Department will be available at the Huron County Public Health office located at 180 Milan Avenue in Norwalk.



Birth and Death Certificates are available at the Huron County Public Health office, 180 Milan Avenue, Norwalk, Ohio 44857. For assistance, please call the Vital Statistics Office at 419-668-1652 (Option 3).

Certificates may be requested by phone, by mail, or in person at the Norwalk office. Certified copies are $25 per copy. Office hours are Monday 9:00 a.m. – 4:30 p.m. and Tuesday-Friday, 8:00 a.m. – 4:30 p.m.

(A Request form may be completed on line and mailed to the Huron County Public Health office, 180 Milan Avenue, Norwalk, OH 44857, along with a check and self addressed stamped envelope, or taken in person to the Norwalk office.)
